A Brazilian butt lift (BBL) takes fat from areas such as the abdomen, flanks or back by liposuction and injects it into the buttocks to change their shape and projection. It is widely advertised by Turkish clinics, often as an all-inclusive package.

It is also an operation in which safety depends closely on technique. The most serious risk — fat entering large veins in the buttock — is linked to where the fat is placed. The questions that protect you are therefore specific and technical: where the fat goes, how the surgeon confirms it, who injects it and how you are monitored. Price comes after those answers.

What a BBL Changes — and What It Does Not

A BBL combines two effects: liposuction slims the waist, flanks and lower back, and fat grafting adds volume and shape to the buttocks. Much of the visible change often comes from the contrast created at the waist.

A BBL does not lift loose, sagging buttock skin; patients with significant laxity after weight loss may need a buttock lift or body lift. It also cannot add more volume than your donor fat and the buttock tissue can safely accept, and not all of the transferred fat survives.

Why Fat Placement Decides Safety

The buttock contains large veins beneath and within the gluteal muscles. Fat injected into or beneath the muscle can enter these vessels and travel to the lungs — a fat embolism, which can be fatal. A safety review published by the British Association of Aesthetic Plastic Surgeons notes that the mortality risk from fat embolism in BBL was previously reported as 1 in 2,500 and later estimated to be nearer 1 in 15,000 — similar to the mortality risk of a tummy tuck. The review links the growing evidence on safety to technology, techniques and training designed to avoid injecting fat into the muscle.

The safety questions therefore centre on:

  • Subcutaneous placement — fat placed in the layer between skin and muscle, not in or under the muscle
  • Intraoperative ultrasound — the BAAPS review reports demonstrated benefits for guiding subcutaneous placement and avoiding inadvertent deep injection
  • Who injects the fat — confirm it is the surgeon you chose
  • Volume and operating time — how much fat will be moved, and how long surgery is expected to take

The same review states that complications are better controlled and managed in strictly regulated healthcare settings, and considers patients using unregulated facilities or medical tourism to be at higher risk of serious complications or inadequate follow-up care. If you are considering a BBL in Turkey, choose a provider who can answer the placement questions clearly and in writing, and confirm how follow-up care will work after you return home.

Is a BBL Suitable for You?

  • Enough donor fat. Very slim patients may not have enough; buttock implants are a different option with different risks.
  • Stable weight. Grafted fat behaves like body fat: it grows with weight gain and shrinks with weight loss.
  • Good general health. Heart and lung conditions, clotting disorders, anaemia and some medications increase risk.
  • Non-smoker. Nicotine reduces fat survival and wound healing.
  • Realistic goals. Surgeons may decline very large volume requests that cannot be placed safely above the muscle.

What Can Change After the Examination

At examination the surgeon assesses donor fat, skin quality, the depth of fat over the buttock muscles and your proportions. The plan may change to:

  • a smaller transfer than you hoped, because the space above the muscle is limited
  • fewer liposuction areas, or staging treatment
  • a different operation altogether if skin laxity is the main issue
  • postponement for blood tests, weight or a health concern

Ask in advance how the price and deposit change if the volume or areas are reduced.

BBL Cost in Turkey

In September 2026, BBL packages advertised by Turkish providers mostly ranged from about €2,200 to €5,800, with many clustered around €3,000–€4,800. Differences usually reflected the number of liposuction areas, hospital stay, anaesthesia arrangements and whether ultrasound guidance and senior surgeons were named.

These are advertised prices, not quotes. A very low BBL price deserves a direct question: what has been left out to reach it — the anaesthetist, ultrasound, hospital monitoring or the surgeon's time?

  1. Will all fat be placed in the subcutaneous layer above the gluteal muscle?
  2. Is ultrasound used to confirm cannula depth during injection?
  3. Who performs the fat grafting — the named surgeon personally?
  4. What is the maximum volume you would consider safe for me, and why?
  5. How long is the operation expected to take, and are other procedures combined?
  6. Who gives the anaesthetic, and how am I monitored overnight?
  7. Where is the operation performed, and is the facility on the HealthTürkiye certified list?

What a BBL Package in Turkey May Include

Depending on the clinic and quotation, a package may include tests, the operation and anaesthesia, one night in hospital, compression garments, a BBL cushion, medication, post-operative checks, hotel nights, transfers and an interpreter. Some include lymphatic massage sessions. Treat anything not written in the quote as excluded.

Before-and-After Photos

BBL photographs are easy to flatter with posture, clothing and lighting. Compare side and back views taken in the same stance, with similar starting body shape, at least six months after surgery. Early photographs show swelling and fat that will partly be reabsorbed.

Risks and Safety Considerations

Beyond fat embolism, BBL risks include fat necrosis and oil cysts, lumps, infection, fluid collections, asymmetry, contour irregularities at liposuction sites, skin necrosis, numbness, loss of volume and blood clots. The BAAPS review also highlights hypothermia and sepsis among serious complications, and poor aesthetic results as a real risk.

Seek urgent medical help for chest pain, sudden breathlessness, confusion, a racing heart or calf swelling, in Turkey or after you return home.

How Long to Stay in Turkey and Flying Home

Patients commonly plan around 7–10 days in Turkey after a BBL. A flight home combines two issues: prolonged sitting on healing grafts and the higher risk of blood clots after surgery. Ask your surgeon how to sit on the plane, whether a cushion or compression stockings are recommended, and how often to walk. An aisle seat helps. The day you can fly is decided by your surgeon after reviewing your recovery.

What is a Brazilian butt lift (BBL)?+

A Brazilian butt lift is a fat-grafting operation that reshapes the buttocks using your own fat. The surgeon removes fat by liposuction from areas such as the abdomen, flanks, lower back or thighs, processes it, and injects it into the buttocks to add volume and projection. Contouring the waist and lower back is often as important to the final shape as the fat added to the buttocks. Despite its name, a BBL does not lift loose skin. Patients with sagging buttock skin may need a buttock or lower body lift instead.

Why is BBL surgery considered higher risk?+

BBL surgery is considered higher risk mainly because of fat embolism, a potentially fatal complication in which fat enters large veins deep in the buttock and travels to the lungs. This risk is associated with injecting fat into or beneath the gluteal muscle. Professional safety guidance advises placing fat only in the fat layer above the muscle, known as the subcutaneous plane. Some surgeons use ultrasound to confirm cannula position. Surgeon experience, careful technique, patient selection and facility preparedness are central to reducing risk, but no BBL can be described as risk-free.

Who is a good candidate for a BBL?+

Good candidates are in good health, at a stable weight, have enough donor fat for liposuction, and want a realistic change in buttock shape. Very slim patients may not have enough fat, while patients with significant loose skin may not get the lift they expect. Smoking, blood clot history, bleeding disorders and some medical conditions increase risk. Plans to lose or gain significant weight should be discussed, because transferred fat changes with body weight. Patients without enough fat may compare this procedure with buttock implants.

How much fat survives after a BBL?+

Only part of the fat transferred during a BBL survives; the rest is reabsorbed by the body over the first months. The amount that survives varies with fat quality, harvesting and processing technique, how the fat is placed, blood supply, smoking and pressure on the buttocks during recovery. Placing excessive volume can increase fat necrosis and other complications, and safe placement above the muscle also limits how much fat the tissue can accept. The buttocks usually look larger soon after surgery than they will later, with the final shape assessed around six months.

What safety questions should I ask a BBL surgeon?+

Ask where the fat will be placed and whether it will remain in the subcutaneous fat above the gluteal muscle. Ask whether ultrasound guidance is used, who personally performs the fat injection, how many BBLs the surgeon performs, the maximum volume they consider safe for you, and how long the operation will take. Confirm the facility type, anaesthesia arrangements, overnight monitoring and emergency transfer plan. Also ask how complications are managed after you return home. What are the other risks of a BBL?+

Besides fat embolism, BBL risks include fat necrosis, oil cysts, lumps, infection, seroma, asymmetry, loss of volume and contour irregularities at the liposuction sites. Blood clots in the legs or lungs, bleeding, numbness, skin discolouration and anaesthesia complications can also occur. Some patients experience irritation of nerves in the buttock area. Larger fat transfers or long combined operations may increase some risks. A new lump or area of firmness after surgery should be assessed by a doctor instead of being assumed to be normal healing.

Can I sit down after a BBL?+

Most surgeons advise avoiding prolonged direct pressure on the buttocks for a period after a BBL, but instructions vary. Pressure may reduce blood flow to the transferred fat and affect how much survives. Patients are often told to sleep on their front or side and to use a cushion placed under the thighs so the buttocks are not pressed when sitting is necessary. Walking regularly remains important to reduce clot risk. Follow your own surgeon's written advice on sitting, driving, compression garments and returning to work, rather than a general timeline found online.

How long should I stay in Turkey after a BBL, and how do I fly home?+

Patients are commonly advised to stay around 7–10 days in Turkey after a BBL before flying. A long flight combines two recovery concerns: pressure on the buttocks and an increased risk of blood clots. Ask your surgeon how to sit during travel, whether a special cushion is recommended, whether compression stockings or medication are needed, and how often to walk. Are BBL results permanent?+

Fat that survives after a BBL can remain long term, but it behaves like other fat in the body. If you gain weight, the transferred fat may enlarge; if you lose weight, the buttocks may become smaller. Ageing, loss of skin elasticity, pregnancy and changes in muscle tone can also affect shape over time. The first months after surgery involve some fat reabsorption, so early size is not the final result. Maintaining a stable weight gives the best chance of preserving the shape achieved after swelling settles.

What affects the cost of a BBL in Turkey?+

BBL cost in Turkey depends on the number of liposuction areas, the amount of fat processed, operating time, the surgeon's fee, facility, anaesthesia and whether ultrasound guidance is used. Compression garments, cushions, drains, tests, medication, follow-up visits and accommodation may be charged separately. An unusually low package price should prompt questions about who performs the injection, how long the surgeon spends on each case and what monitoring is provided.
